Please download the Group Registration Form, fill in all participant details, and email it to the conference secretariat.
Once your form is received, the secretariat will issue an invoice. After payment is made, confirmation emails will be sent to all participants.

Payment Methods

  • Online Banking: Payable to the Malaysian Diabetes Educators Society. Invoice and payment details will be provided after registration.

  • LPO: For payment via Local Purchase Order, please contact the conference secretariat

Name Change/ Substitution

All registrations are only valid for the registered person. A registered delegate who is unable to attend the conference may nominate (free of charge) a substitute delegate by notifying the Conference Secretariat in writing by 15 April 2026 with complete information as required in the online registration form.

Cancellation Policy

Any cancellation of registration should be communicated in writing and will be subject to the following conditions:

  • Cancellation on or before 15 April 2026: 75% refund.
  • Cancellation after 15 April 2026: No refund will be provided.

  • The Committee reserves the right to change the date(s) or speaker(s), if deem fit, without prior notice.
  • The Committee further reserves the right to cancel/postpone the event without liability other than return of the course fee.
  • The organiser is not responsible for any loss or damage as a result of substitution, alteration, postponement or cancellation of an conference.
  • At this conference, photographs and videos may be taken by the event organiser for promotional and documentation purposes. By attending this conference, you grant permission for your image to be used in promotional materials such as brochures, websites, and social media by The Organiser; unless advised by you otherwise.
